The 2026/27 pay deal: how Scotland does it differently
Scotland does not use the NHS Pay Review Body. Instead, the Scottish Government negotiates Agenda for Change pay directly with trade unions — and for this round it agreed a two-year deal covering around 170,000 NHS Scotland staff, worth roughly £700 million. - Year 1 (2025/26): 4.4%. The original offer was 4.25%, but the deal's inflation guarantee was triggered: the guarantee promises pay rises at least one percentage point above average calendar-year CPI, and with 2025 CPI confirmed at 3.4%, an extra 0.15% was added. The uplift was paid in February salaries with arrears in March 2026.
- Year 2 (2026/27): 3.75%, effective 1 April 2026.
- The same CPI-plus-one-percentage-point guarantee applies to the 2026/27 figure and will be rechecked in early 2027 — so if inflation runs hotter than expected, Scottish staff could see a further adjustment.
NHS Scotland pay scale 2026/27
Scotland publishes its own Agenda for Change pay scale, separate from the English one. Verified figures for 2026/27 include:

Scottish hourly rates are calculated on the new 36-hour week (annual salary ÷ 52.2 ÷ 36). The 36-hour week: what changed in April 2026
From 1 April 2026, full-time in NHS Scotland means 36 hours a week, down from 37.5. This matters in three practical ways:
- Same annual salary, fewer hours. Because pay was not cut, the shorter week is effectively an hourly pay rise on top of the 3.75% uplift. A Band 5 nurse at the top of the band now earns £22.87 an hour in Scotland, compared with £19.95 an hour for the equivalent role in England.
- The overtime threshold moved. Overtime rates now kick in above 36 hours a week instead of 37.5, so extra shifts qualify for enhanced pay sooner.
- Rostering and part-time contracts were adjusted to the new standard week — if you work part-time, check your contracted hours were recalculated correctly, as this affects your pro-rata salary.
Scottish NHS pension tiers
Scotland runs its own NHS pension contribution tiers, separate from England and Wales. For 2026/27 there are nine tiers, with employee contributions ranging from 5.7% to 12.7% of pensionable pay depending on your whole-time-equivalent salary — compared with six tiers (5.2% to 12.5%) in England and Wales. The employer contribution rate is 23.7% of pensionable pay.
Because Scottish salaries are higher at equivalent bands, some staff sit in a higher contribution tier than their English counterparts — worth checking on your payslip, as the tier is based on your notional whole-time salary even if you work part-time.
Scotland vs England: the full picture
Pay is only part of the story. When comparing NHS Scotland with NHS England, factor in:
- Higher headline pay at equivalent bands, plus the 36-hour week.
- Different income tax bands. Scotland sets its own income tax rates and thresholds, which affects take-home pay — higher earners in particular can take home less than the gross figures suggest. Run your salary through a Scotland-specific take-home calculator rather than a UK-wide one.
- Annual leave follows the same Agenda for Change structure (27 days rising to 33, plus bank holidays, pro rata for part-time).

What the 2026/27 rise means for your take-home
A 3.75% rise on a £34,494 Band 5 entry salary adds about £1,294 a year before deductions. What lands in your bank account depends on your income tax band (Scottish rates), your pension tier, student loan deductions and any unsocial-hours enhancements. Because the rise was effective 1 April 2026 and paid on time, there was no backpay to complicate 2026/27 payslips — unlike the 2025/26 award, which arrived with arrears.
