NHS Band 7 salary 2026/27
The 2026/27 Agenda for Change award added 3.3% to every pay point in England. It was announced on 12 February 2026, backdated to 1 April 2026 and paid on time in April salaries — the first on-time award in six years (NHS Employers). Hourly rates are calculated as annual salary ÷ 52.2 weeks ÷ 37.5 hours, the standard NHS full-time formula. For comparison, 2025/26 Band 7 ran from £47,810 to £54,710. What jobs are Band 7 in the NHS?
Band 7 is the first true leadership band for most clinical professions — the level where you run a ward, lead a team or practise with advanced autonomy. Well-established Band 7 roles include ward manager, advanced nurse practitioner / advanced clinical practitioner, team leader midwife, operational service manager, senior sister / senior charge nurse and clinical educator lead.
Posts at this level expect proven leadership: managing staff, owning budgets and rotas, and accountability for service quality. Advanced practice roles typically require master's-level education and independent prescribing.
Band 7 take-home pay and pension
From gross pay come income tax, National Insurance, NHS pension contributions and, for many, student loan repayments. Band 7 straddles two pension tiers, which catches some staff out: at entry (£49,387) the contribution rate is 9.8%, but from £50,846 it rises to 10.7% (official 2025/26 thresholds; 2026/27 thresholds are provisional and expected to move slightly). As a guide: at entry, pension contributions are around £4,840 a year (roughly £403 a month before tax relief); at the top of the band (£56,515) they are around £6,047 a year (roughly £504 a month). The employer contributes 23.7% on top regardless of tier. Note that all of Band 7 sits above the £50,270 higher-rate income tax threshold at the top end, so staff at the top pay 40% tax on earnings above that line.
Overtime and unsocial hours at Band 7
Band 7 is the last band that qualifies for paid overtime: hours above 37.5 per week are paid at time-and-a-half, rising to double time on bank and public holidays (bands 8 and above receive time off in lieu instead). Part-time staff are paid plain time for additional hours up to 37.5 hours.
Unsocial hours enhancements (Section 2 of the AfC handbook) apply at Band 7: time plus 30% for weekday nights (8pm–6am) and Saturdays, and time plus 60% for Sundays and public holidays. Band 7 pay in London: HCAS
Staff in and around London receive a High Cost Area Supplement on top of basic pay. For 2026/27: inner London 20% (minimum £5,794, maximum £8,746), outer London 15% (£4,870–£6,137) and fringe 5% (£1,346–£2,270). HCAS is pensionable.
Example: a Band 7 ward manager at the entry step (£49,387) in inner London would receive 20% HCAS — £9,877 — but this is capped at the £8,746 maximum, taking total pay to £58,133. How to progress from Band 7 to Band 8a
Within Band 7 you move up the pay steps each year, subject to the annual appraisal process. Reaching Band 8a means winning a Band 8a post — typically a service manager, lead specialist or assistant general manager role. The jump is as much about scope as seniority: Band 8a posts usually span multiple teams or a whole service, so build evidence of cross-team leadership, business planning, budget management and service redesign. Postgraduate leadership or management qualifications strengthen applications considerably.
