NHS Band 8d salary 2026/27
The 2026/27 Agenda for Change award added 3.3% to every pay point in England. It was announced on 12 February 2026, backdated to 1 April 2026 and paid on time in April salaries — the first on-time award in six years (NHS Employers). England uses three pay steps per band (entry, intermediate, top), with spine points abolished under the 2018 AfC deal.

Hourly rates are calculated as annual salary ÷ 52.2 weeks ÷ 37.5 hours, the standard NHS full-time formula. For comparison, 2025/26 Band 8d ran from £91,342 to £103,433. All figures are for a full-time 37.5-hour contract; part-time staff are paid pro rata (Health Careers NHS).
What jobs are Band 8d in the NHS?
Band 8d is the highest sub-division of Band 8 — director-level roles sitting just below the executive board and the Band 9 tier. Well-established Band 8d roles include director of nursing or midwifery at smaller trusts, director of allied health professions, divisional director with the largest portfolios and deputy chief nurse or deputy chief operating officer posts.
These roles carry some of the heaviest accountability in the NHS: whole-workforce leadership, patient safety across divisions and direct responsibility to the chief executive. They are typically reached after a distinguished record at Band 8c, often with national profile in the profession.
Band 8d take-home pay and pension
From gross pay come income tax, National Insurance, NHS pension contributions and, for some, student loan repayments. The whole of Band 8d sits in the top NHS pension tier: 12.5% of pensionable pay (official 2025/26 threshold of £65,191 and above; 2026/27 thresholds are provisional and expected to move slightly). As a guide: at entry (£94,356) pension contributions are around £11,795 a year (roughly £983 a month before tax relief); at the top of the band (£108,814) they are around £13,602 a year (roughly £1,133 a month). The employer contributes 23.7% on top regardless of tier. All of Band 8d is well above the £50,270 higher-rate income tax threshold, so earnings above that line are taxed at 40%.
Overtime and unsocial hours at Band 8d
There is no contractual overtime pay at Band 8d. Extra hours are compensated with time off in lieu (TOIL), subject to local agreements — at this level, the working pattern is dictated by the demands of the role rather than the clock. Unsocial hours enhancements (Section 2 of the AfC handbook) technically still apply to bands 4–9: time plus 30% for weekday nights (8pm–6am) and Saturdays, and time plus 60% for Sundays and public holidays. In practice, Band 8d roles are not rostered for unsocial hours.
Band 8d pay in London: HCAS
Staff in and around London receive a High Cost Area Supplement on top of basic pay — see NHS London weighting for the full HCAS rates and zones. For 2026/27: inner London 20% (minimum £5,794, maximum £8,746), outer London 15% (£4,870–£6,137) and fringe 5% (£1,346–£2,270). HCAS is pensionable.
At Band 8d salaries the inner London percentage is always capped: 20% of the £94,356 entry salary would be £18,871, so it is limited to the £8,746 maximum, taking total pay to £103,102. Every Band 8d post in inner London receives the maximum HCAS.
How to progress from Band 8d to Band 9
Within Band 8d you move up the pay steps each year, subject to the annual appraisal process. Band 9 is the summit of Agenda for Change — a small number of posts such as chief nurse, director-level roles at the largest trusts and a handful of national clinical leadership positions. Progression means competing at open national recruitment against the strongest candidates in the country. The differentiator is a career-defining record: leading organisations through their hardest periods, national policy influence and the confidence of ministers, regulators and system leaders.
